In crisis situations, our services allow timely and informed decisions. While there are many types of assisted living communities, once medical needs, finances and personal preferences are taken into account, choices become limited. We want you to feel confident all avenues have been explored before making such an important decision.
Transitioning to assisted living is one of the most emotionally difficult and challenging times both you and a loved one will face. We care for your family just as we do our own. From our first interaction and throughout the transition we are here to provide you with support and resources to help ease some of the stress and confusion that accompany this process. Below is information our Placement Consultants will discuss in order to begin the process of finding the best assisted living options.
General Education About Assisted Living: There is an endless amount of information about assisted living on the Internet. In addition to discussing the way the system works, our Placement Specialists can help you make sense of the information you’ve already gathered. Information is only useful if you understand and know how to utilize it!
Specific Information About Individual NH Assisted Living Communities: After awhile you may find it difficult to distinguish one community from another. They are however very unique. Differences would include such things as Medicaid Policy vs Private Pay Policies, Rate Structure, Level of Medical Care Offered, Dementia Care Programs, Types of Activities, Amenities, Culture of Residents, Physical Characteristics of the Community, Personal Living Quarters, etc.
Identification of Assisted Living Options: We make sure you know all your options. Based on your loved one’s Level Of Care, Finances and Preferences, we are able to identify which communities have the best ability to meet their needs.
Level of Care: The level of medical and non-medical care required by your loved one is the first step in identifying an appropriate community. While both residential assisted living and assisted living communities will assist with Activities of Daily Living (ADLs), some are limited in their ability to provide medical care. The medical care a community is able to provide is determined by state licensing along with the policies set by the individual communities. Injectable medications, liquid oxygen and mobility or incontinence issues or a diagnosis of dementia will limit options.
Finances: Once we establish which communities can meet your loved one’s medical needs, we need to determine those within your budget. Residential Homes for the most part are limited to providing assistance with ADLs and average $2800 a month for a bedroom. Assisted Living Communities provide varying levels of medical care in addition to ADLs. The average ranges from $4,200 to $7,000 dependent on the size of the apartment and level of care required. Based on medical and financial eligibility your loved one may qualify for Federal (Veteran’s Benefits) or State assistance (Choices For Independence).
Personal Preferences: Once we have determined the communities able to meet your loved one’s Level of Care within the determined budget, we can begin to talk about the personal preferences. They can make all the difference when it comes to making their new home, feel like home. Some examples are: Pet Policy, Smoking Policy, Availability of off-site shuttle, Resident Culture, Designated Medical Care Facilities and Physicians, etc.
Coordination of Tours: Our staff has multiple direct contacts at the various communities allowing for quick response to our calls. In crisis situations we are able to quickly identify and coordinate tours at communities that have the best ability to meet your loved one’s needs.
Provide Resources: In addition to financial resources, we have built relationships with various types of agencies whose services are tailored towards seniors. They may be able to assist in making the transition to assisted living easier. Our resources page talks about various resources you may find helpful.

The final decision of where a perspective resident should reside is made by the family, caretaker or responsible party and should be based on information provided directly from the community. Whether a community is able to meet the medical needs of the perspective resident is determined by the assisted living community’s assessment.
