Meet with Patient
For those with time sensitive placements, they become our priority. We offer an assessment of your loved one’s needs within a 24hr window.
Consult with Care Team
Care Team and/or Discharge Coordinator will provide us with an outline of the medical and cognitive support that will be needed upon discharge. They will also drive the time frame for discharge- dictated by Medicare.
Set-Up Assessment
We can quickly locate appropriate communities with openings, followed with tours and assessments by a community’s nursing staff.
Admission Paperwork
Overseeing the admission paperwork gets completed correctly and into the right hands is another safety net to a smooth transition into a community. And, making sure arrangements for transportation from the hospital to community has been set in motion!
